(SHERWOOD, Ore.) — Authorities identified the woman killed after being attacked by a cougar at an Oregon wild cat sanctuary on Saturday evening.

Rene Radziwon Chapman, 36, may not have followed safety protocols prior to her death as she was was alone in the enclosure at the time of the accident.

WildCat Haven Sanctuary’s policy dictates that when working with dangerous animals, there must be two qualified staff members present at all times.

First responders were concerned about entering the park, not wanting to allow animals to get out, and that they would be unable to locate some of the larger and more dangerous animals.

WildCat Haven Sanctuary is a “last hope” facility for more than 60 captive-born wildcats and hybrids that have been neglected, abandoned and abused, according to its website. The facility is not open to the public, but provides on-site tours for donors.
